All Care Health Center began in 1997 as a clinic located at Place 35 in the west end of Council Bluffs. In 1998, as the need for services grew, the clinic was relocated to the Omni Center as a Federally Qualified Health Center (FQHC). Since then a Dental clinic, X-ray services, a handicapped accessible van and translation services, and certified application counselors have been added to eliminate barriers to health care for the entire community. All Care Health Center went by the name Council Bluffs Community Health Center from 1998 until 2012.
