Canyonlands Healthcare Clifton

Canyonlands Healthcare (CHC) is a 330e community health center organization serving frontier populations located in northern and southeast Arizona. Canyonlands was established in 1973 with Lake Powell Medical Center and has grown to include seven community health centers located in Page, Beaver Dam, Chilchinbeto, Fredonia, Duncan, and Safford. CHC’s mission is to provide accommodating, affordable and accessible health care.

CHC serves growing rural populations residing in Mojave, Coconino, Navajo, Graham, and Greenlee Counties, and focuses on increasing access and reducing health disparities among these populations. These services are provided to individuals and families in some of the most remote and isolated areas in the along state borders. For that reason CHC serves not only residents of Arizona, but also residents of Utah, Nevada, and New Mexico.

CHC health centers are valued entities in these communities and those communities that surround them. CHC provides a broad spectrum of primary, preventative and enabling services to these medically underserved populations, including prenatal, general adult medical, obstetrics/gynecology, pediatric services, counseling, and oral health services. In addition to these health care services, CHC provides health education, translation, and case management.
