Intercept Youth Community

Established in 1996, Intercept has helped countless At-Risk youth and families to achieve success in their daily lives, while earning awards for service excellence. Regarded as an innovator in the field of Human Services, Intercept is proud to offer LifeBridge, a philosophy and a system of care that establishes the best pathway to permanency by offering the “right service at the right time” for troubled youth, young adults and their families. LifeBridge affirms that all youth and young adults should be treated within the community, transition to less intensive services provided by familiar faces, and maintain the goal of permanency. As a system of care, LifeBridge provides a complete continuum of community based services designed to cover the full spectrum of needs of At-Risk youth. Most importantly, this system coordinates the movement of youth through these services by utilizing a “Community Integration Plan” through which all treatment and case management is driven.
LifeBridge is a true continuum, providing a seamless transition between services where relationships between the youth, family and Intercept counselor endure. Whether used individually or as a continuum, Intercept’s array of services results in the timely and successful rejoining of youth with their natural family, foster / adoptive family, or in their transition to independence with permanency supports.
