Elite provides services to include: Care Companions/Homemakers, Home Health Aides/Certified Nursing Assistants, Medication Aides, Skilled Nursing, Physical Therapy, Speech Therapy, and Occupational Therapy, Medical Social Work.

**Our goal at Elite Professionals Home Care Co. is to assist each individual client in maintaining their independence and provide them the opportunity to continue to enjoy their desired living situation. We work closely with our client’s physicians to provide the highest quality of care possible. Each client will receive individualized and superior levels of care.

Care Companion Program:
Our Care Companion Program is unique to those clients that do not require hands on cares and request only housekeeping, laundry, meals, running errands, transportation to appointments, or simply companionship.

Benefiting our patients:
This program is unique in that Elite can transition our clients into a higher level of care to include bathing or skilled care as their health needs increase. This saves our clients from having to switch home health companies or caregivers when those needs arise.

Home Health Aide Services:
Our staff members are available for all activities of daily living to include: bathing, dressing, transferring, etc. All of Elite’s HHA staff are also medication aides so that they are able to assist with medication administration in the home! All HHA cares are guided by an RN-Case Manager staff member and are ordered by the patient's physician.

Nursing Cares:
Elite provides both low-tech and high-tech nursing to our clients. Low-tech nursing includes but is not limited to: medication planner fills, wound care, IV administration to include TPN/ IVIG/ antibiotics/etc, urinary catheter cares, plus much more! Our high-tech nursing includes but is not limited to: ventilator and tracheostomy cares. Elite also has a specific Behavioral Health Program to include Psychiatric Nursing for those patient's with specific behavioral health needs.

Therapies:
Elite provides Physical, Occupational, and Speech Therapies in our clients home upon physician request!

Speech therapy involves more than correcting pronunciation. Difficulty swallowing, slurred or slowed speech, and other communication issues are common results of an individual who has experienced a stroke or other chronic condition. is especially important to those who have experienced a stroke or chronic conditions that cause difficulty swallowing and articulating their words.

Our Speech Language Pathologists (SLP) are trained in the following areas:

Swallowing evaluations
Nutritional assessment & education
Language skill recovery
Speech production
Cognitive functioning
General communication
Tube feeding assessment and education

The goal of our Occupational Therapists (OT) is to provide treatment while maintaining our patients’ independence and dignity. Our treatment plans are entirely customized to the individual to ensure his or her primary obstacles are addressed and overcome.

Our OTs are familiar with helping patients with the following tasks:

Getting dressed
Taking a shower
Cooking meals
Writing
Grabbing small objects
Opening cans
And more

Newly added CLT Program: Our Certified Lymphedema Therapists are trained in varied lymphedema therapies including:

Manual lymph drainage
Intermittent compression therapy
Multi-layered bandaging, including home instruction
Custom lymphedema garment fitting
Therapeutic exercises to promote lymph drainage
Education for patients and families on the condition and prevention methods
Patient advocacy for lymphedema awareness, prevention and treatment

Our Physical Therapy staff understand the importance of recovering at one’s own pace. With our staff in the home, we can help reduce risk of falls while promoting improved mobility and functionality. Individuals who have issue with any of the following can benefit from our physical therapy treatments:

Medical Social Worker:
Patient advocacy is an important piece to our home care programs. Our medical social workers have worked with patients and their families in various medical situations, including coordination among healthcare providers, end-of-life preparations, and grief counseling. Our MSW have intimate knowledge of the United States' healthcare system and here to help navigate life's most challenging moments.

Medical Social Work visits include but are not limited to: Assessment of Homebound status, Medications, Food security, and Social Determinants of Health. Social Determinants of Health assessment completed and outlined patient’s identified risks. Identified risks will be listed in documentation - will refer patient to appropriate community resources as available.

Elite is licensed in the State of Nebraska, is Medicare and Medicaid certified, and accepts long-term care insurance.

Payment for Services:
Elite accepts multiple forms of Insurance/payment. Some of those payment forms include the following: Medicare, Medicaid, Medicaid Waiver, Insurance (PPO/HMO): BCBS, Coventry, Aetna, United, Humana, Midland’s Choice, TriCare, etc, Long-Term Care Insurance, VA, multiple city & state funded programs, Private Pay, etc.

**Elite provides background checks, APS/CPS checks, sex registry checks, and reference checks on all of its employees. Elite carries Liability and Workman’s Compensation insurance on all our employees.

** In order to meet our patient’s needs Elite’s RN staff members are on-call 24 hours per day, 7 days per week!
