MBK was first established in 1999 by Dr. Mark Anthony Colomb as a nascent entity dedicated to the prevention, care, and treatment of persons infected with HIV. Through its extensive experience, MBK built its commitment to HIV prevention among racial and ethnic minority populations as evident through its service to over 15,000 constituents nationwide, who represent community-based organizations, faith-based organizations, institutions of higher learning, and community lay persons.

As it exists today, MBK is an experienced community-based organization working to eliminate health disparities among underserved, uninsured/underinsured, and other vulnerable populations through health promotion, policy and environmental systems changes, and other health equity approaches. MBK's efforts have expanded to include a wealth of health issues, including cardiovascular disease, diabetes, hypertension, obesity, and cancer, which has systemically transformed MBK from a grassroots organization into a community-based leader that strives to change the health status of Americans one community at a time.

MBK's organizational structure consists of three complementary Centers designed to create an integrated services approach to the prevention, treatment, and care of HIV/AIDS, STIs, and a host of other chronic conditions, They are: Center for Community Based Programs (Ridgeland), Center for Research, Evaluation, and Environmental & Policy Change (Jackson), and MBK South Gulf Coast (Hattiesburg and Gulfport-Biloxi). Its clinical arm includes Open Arms Healthcare Center, a community clinic in Jackson offering holistic primary care services established in 2013.
