Health Quality Partners (HQP) designs, tests, and refines improvements in health care delivery to achieve better health outcomes for vulnerable populations. The HQP program of community-based care management is one example of a new health design concept developed by HQP; the advanced preventive service. The model has been successfully tested in the Medicare Coordinated Care Demonstration and shown to reduce hospitalizations, ER visits, cost, and mortality among higher-risk chronically ill older adults. HQP is also serving high-risk Medicare Advantage members enrolled with Aetna in southeast Pennsylvania.
