Urban Health Plan (UHP) is a network of federally qualified community health centers based in the South Bronx, Central Harlem and Queens and has been serving the community since 1974.
We have ten clinical sites, one mental health facility, twelve school-based sites and three part-time sites. In 2018, we served more than 87,000 patients. UHP is Joint Commission accredited.
