Not-for-profit Hospice of San Joaquin provides care when cure is exhausted.
Hospice of San Joaquin has been in San Joaquin County for 39 years and is Joint Commission accredited.
Through guidance from an interdisciplinary team, families and caregivers learn to adapt to changes in the health of their loved one. Hospice of San Joaquin care is provided in whatever setting the patient calls home, as well as our Hospice House.
Services include: visits from the team members, nurses, social workers, home health aides, in addition to medication and medical equipment needed for the patients hospice diagnosis. Spiritual support, and caregiver education, are also provided. Bereavement support is offered for 1 year following the loss of a loved one (also offered as a community service to anyone who has lost a loved one).
Continuous Care, Respite care, and General Inpatient care are all services that HSJ provide as well.
