The first Siletz Community Health Clinic was opened on February 9, 1991. The building we occupy now was completed in April, 2010 and offers a range of outpatient services including: Family Practice, Pediatrics, Pharmacy, Laboratory, Radiology, Dental, Optometry, Diabetes Education, Community Health, Prevention, Alcohol and Drug and Mental Health Care. All of our practitioners participate in continuing education requirements related to their licensure and credentials.
We provide quality healthcare, involving you in decisions about your health, and participate with you in developing and maintaining a healthy lifestyle for the future.
