Dr. Cook is a 1976 graduate of Sidney High School. He received his Bachelor of Science degree in 1979 from the University of Houston with a degree in Physiological Optics. In 1983, he received his Doctorate of Optometry from the University of Houston. His residency was performed at Fort Carson Army Hospital, where he received numerous awards for his performance.
He opened his solo practice in 1983 in Sidney, Nebraska. He is a member of the Nebraska Optometric Association and the American Optometric Association.
Dr. Cook is certified in Pre and Post Lasik Surgery Treatment and has lectured on numerous occasions on the affects of diabetes and the eye.
