This initial visit allows us to better understand your medical and dental conditions and needs. To assist us, we ask that you provide accurate, up-to-date medical information, including a list of current medications, a completed referral card from your dentist, medical and dental insurance information, and appropriate x-rays. If additional x-rays are needed, we use digital x-ray technology which significantly reduces radiation exposure by as much as 90%.
